I don't know if this is the same survey that was proposed during a board 
meeting, but I recall volunteering to participate...  and this is the first 
I've seen of a draft.  Given that surveys are one of my responsibilities with 
the government, I thought that I could, at least, provide some advice.

Question 7.  multiple response is possible.

Question 8.  how about ads in the Coast?  

Question 9.  this type of question sets the reader up, biasing the response.

Each question in a survey must serve some purpose - what is the question 
attempting to measure, and some relationship to the whole.  For example - the 
survey has one question on demographics (age) - an interesting statistic to 
have - but it bears little relationship with the other questions.  Does it 
serve any purpose to know that an 18-25 is using a 486?

Anyway... just a couple of thoughts...
