Are home directories mounted to various systems from a central server?

If so then if the NFS home directory server is under heavy load or is not
responding for any other reason the mail system on the NFS client might
not be able to open the .forward file. Depending on exactly how your email
system is setup that could result in email being placed in the users system
mailbox rather than being forwarded.

To avoid problems like this your mail server should be the same system which
holds user home directories. All auxiliary mail systems should forward mail
to the main mail server for things like alias and .forward expansion and
local delivery.
